Ichijo fell into the mistake of 'I figured it out, there's nothing beyond that.' Once he decided that Sakazaki attacking the machine was about Kaiji robbing the vault, he didn't bother to check if they were up to other things. Classic 'smart guy always surrounded by dumb guys goes up against another smart guy' moment.
