At the doctor's office I am sure the doctor could help Aiko's niece, Rika Chan, voice to be naturally an octave higher than Raku for several hours with him having to concentrate. It would be just another thoughtful thing she does for her niece so Rika doesn't fall out of character accidentally if a customer is impolite. But i expect Rika will have more medicine beyond her vitamins to take. Does Rika Chan translate in HELLO KITTY or maybe alcohol. If the latter she could get a cute nickname of an expensive or popular alcoholic drink.
