It was made clear that he had to learn what pain was and ever since he learned pain he can feel it and understand it. Obviously he can heal immediately but ever since he learned what pain was in the beginning of the series he actually feels it. Maybe he was confused. Nothing new lmao. I do understand the confusion a little though since some of the time he literally doesn't react or handles it in a sufficient way. It is a little weird with seeing him act like he feels it and seeing him not act like it at all.
