It's interesting that they decided to adapt this little side story when, if I recall correctly, this wasn't in the OG anime. They already teased the Kyoto arc when Shishio was mentioned in the last episode... so most likely they really might end the series on a cliffhanger, considering we've only got 4 episodes left, of which next episode is expected to conclude this little side story.

This actually was a special prequel Manga that wasn't part of the main original Manga run that came out at around the same time as the second live action movie, they decided to just go ahead and adopt it here.

Don't know if you caught it, the reason that guy doesn't want the masked doctor treating people for free, is it will encourage all the other doctors to cut their prices to compete and he won't be able to charge through the nose.
He also can't directly attack the Doctor because she's a foreigner and it might cause an international incident. (In History when a Foreigner had been harmed by Japanese, the country that Foreigner belongs to often has Japan find out for fucking around. And this city benefits from Western trade in technology, goods, and culture.) So he was going to have his men harm the sick women instead.
Yes on Kenshin info about major historical events and places is usually 100% real, some of the personal lives of the figures (like Sano's old captain) are artistic license but a lot of the past events in Rurouni Kenshin is historical.
