I don't know if you saw this episode's uncensored scene at this point, but in the novel what Ryunosuke was doing was cutting the child's belly open and made a string instrument with their intestine. The music was the scream of the victim. Such a sick duo. Another thing that I could tell you is that servants can sense other servant's present even in their astral form. Assassin class has a passsive ability 'Presence Concealment' so other servants can't sense them easily. That's why other servants aren't noticing them.
