I don't think all of them use that Pod, what I am getting from it is that he is basically on life support. But because of the advanced tech he is not in a full Coma but in a some kind of awake state while not being in any state to communicate or really notice what is going on. And I personally thought that the smile he has... is not "haha look he is smiling" but more like a "wtf, that smile is creepy and gets even creepier" especially in the 2nd shot where he mouth is a bit open it has more something of a crazy/insane smile. So who knows what he experienced. But yeah, the world building is pretty crazy in this show and just throws more and more questions at you.
