Tohru is a mirror of Akito. The first comment sort of explains it but here: This is sort of putting in in more simple terms but Akito didn't want others to abandon her, because it would have made her a "mistake". Tohru didn't want to be abandoned by her mother (if you want to look at it deeply you could also see that as her being a "mistake") The difference between them is Akito had a selfish and unloving mother and Tohru had the opposite: a selfless and loving mother. This is my interpretation, however, you could see that as sort of the "nurture" side of their characters, as it creates their defining personality traits. Tohru, trying to live the way her mother "wished" decided to completely reject herself and became utterly selfless to an unhealthy degree. You got angry at the knife scene, but I think it's because Tohru literally doesn't care if she herself dies. Akito, having the opposite childhood, became completely selfish. She prioritized her feelings above everyone and blamed others for her own actions. However, in the end, they are two very similar characters.

Why when she is hurt the most it cames so eacy to her to focus on others insted. Its a respons and strategi she has used all her years. That is the way she has coped with her trauma of first losing her dad (& "mom"), and that strategi has just been reinforced and supported by her mom since early childhood etc. Compare that to someone who "deals" with their own trauma and feelings by throwing it on to others, disregard others feelings to only focus on themself. Another strategi that also was reinforced by her mom and other attendence since early childhood etc.
