Something I will point out about Al is that he makes a great psychological case study. He is still perpetually the same age as he was when they tried human transmutation however he still has memory and can accumulate experience so in that sense he is able to grow, but it's impossible to really know how much that experience changes you if your mind never physically develops with your experience. It's not unreasonable to say he probably would still have a childish fascination with the concept of giving birth and new life.
