Not sure if you brought this up in the old discussions, but one thing to keep in mind about Monster is that the length of a manga chapter and an anime episode are usually not equivalent. I would say that a chapter is generally about 0.7 of an episode. A side effect of this is that the anime often squeezes 2 ch into 1 ep (like this one), or stretches 1 ch to fill an entire ep (like Old Soldier and Young Girl and The Fifth Spoonful of Sugar). This can create some potential pacing issues that the manga doesn’t have, as well as possible abrupt tone shifts in the middle of an ep that were not a creative decision by Urasawa. Not saying this worsened the experience of this episode compared to what it could have been, tho it might have a bit... But, well, those sorts of things are inevitable when adapting to another medium.
