Based on what I read I don’t think this is unique to Japan 🥲 my country also struggles to take care of orphans. And I’m sure there are many more than Japan has (statistically)

Japan has a social stigma regarding the adoption of orphans. Also, in episode 159/chapter 449: when Kakashi is talking to his dad in limbo, Sakumo says "What a shame both you and I would die such early deaths...though not as early as your mother" The presumption would be that she died in childbirth.
