To be fair to Otto, the White Whale's screams are said to drive people insane. Remember the whale's screams made soldiers claw at their own eyes out during the battle. And since Otto can understand animals (he explained this at one point) he especially may be liable to going insane from hearing it and inclined to pushing Subaru off the carriage.
