So, the old man by Title is the 3rd Hokage. He retired and passed the on the title to the 4th hokage, but he died early (the opening scene of the series). Choosing a new hokage takes time, and villages can attack if they find out there’s no hokage, so rather than take the risk, Sarutobi had to come out of retirement. That’s why they still call him Lord Third, rather than 5th.
