Yeah, when you laughed at Clay for showing obvious pain after punching Darby in that cell... yeah, his hands are all ate-up with arthritis.
He really should be out of the game, but he won't, because of power. That's kinda the whole through-line of this series. Loyalty, Brotherhood, Family by blood and by affirmation and the surprising strength of those bonds and what people will do to honor them... and just how corrosive Power can be to those same bonds.

Ha, I think some, if not most of these characters are written to be unlikable. And I like that. Hamlet, remember?
Also, cortisone shots are a steroid shots into a joint or tendon, to reduce inflammation, and provide some longer term pain relief. Sometimes the shots have a little pain reliever or local anesthetic mixed in to further relieve pain. It's a double edged sword though, as you get slightly diminishing returns with each shot. Also there's a limit to how many you get, because they actually start to damage cartilage. Source: Me. Played 2A college ball, and have had like 13 knee surgeries, including both replaced. But seriously, when it works, it's like magic.
