We do see how the Major has different ways of holding on to her identity in the later episodes, so I can understand why she'd always use the same model too.

As for why the Major dresses the way she does, it could be like 2B... but I think we got a good hint back when Batou said to her that she ought to get a male model for more power. She gave him a cute smile and hacked his arm so he punched himself. When people see a pretty lady, their reaction is often predictable. Predictable reactions (and distractions) can assist the Major in hacking their cyberware. And, of course, it's "her"; it's what she looks like. Body dysmporphia is probably a problem with full-body replacement prosthetics. When she has a choice, she seems to want to pilot bodies with her hair colour and general facial features.
