I didn't put together at first that Astarion is uncomfortable, though I did wonder why he looks to the side. I guess it's because it's how I cuddle, whenever someone holds me I literally hold them back. But makes sense, the vulnerability of being naked and hold on his neck/chin can make anyone uncomfortable, especially whenit'sb triggering traumatic memories.
