She did have to heal broken bones and heavy damage caused by brutal handling but didn't do much else to ease Karas suffering. There's a lot she did within that year I don't outline cause its a bit extreme but she really did torture Kara. Kara changed dramatically cause the way she lived for a year rewired her brain through trauma responses.
